How SynCare™ Works

SynCare™ is a subscription-based telehealth service for monitoring your stroke and cardiovascular health. We use smart devices to monitor daily blood pressure and other activities along with your health history.

Onboarding & Initial Visit

All plans begin with onboarding including patient portal setup, medical history screening, smart device shipping, and an initial telehealth visit with a board-certified neurologist.

Ongoing Monitoring

After onboarding, measure your blood pressure daily or as indicated by your provider using the smart blood pressure monitor provided. Our team will monitor your data and alert you if an intervention is needed.

Additional Devices

You may connect additional devices such as a smart watch or smart pillbox to give us more insight into your health and progress. See our list of approved devices. Pricing for additional devices varies by plan.
