FOR STROKE SURVIVORS
Similarities to typical Neurology stroke follow-up
Neurologists verify stroke etiology and implement etiology specific treatment
Neurologists evaluate patient symptoms and document the neurological exam during patient encounters
Clinical team personalizes management of key vascular factors according to AHA guidelines which may include:
Adjusting or prescribing statins, antiplatelets, anticoagulants, or antihypertensives
Placing referrals to other specialists
Ordering diagnostic tests
Differences from typical Neurology stroke follow-up
Patient encounters performed over video; all care is remote
Multidisciplinary team with OTs that evaluate home safety and unmet rehab needs (do not do teletherapy)
We take ownership of the patient’s blood pressure management given:
Real time access to dynamic BP data
Can correlate BP patterns with patterns in patient physiology, meds, behavior from assessments and device data other than home BP device
Our clinical team is able to act more rapidly since they are connected with the patient on our app
FOR PATIENTS AT RISK OF FIRST TIME STROKE
Perform comprehensive screening and identify which active risk factors contribute most to an individual’s risk of future stroke and cardiovascular disease
Closely monitor medication adherence, personalize a home exercise plan and diet based on risk factors and comorbidities, and take ownership of the patient’s blood pressure management
All care is in accordance with the AHA guidelines for primary stroke prevention
Patients with any of the following conditions may be at a higher risk of stroke:
Hypertension
Hyperlipidemia
Carotid stenosis
Atrial fibrillation
Obesity
Obstructive sleep apnea
Congestive heart failure
Chronic kidney disease
Family history of stroke
